WORTHINGTON -- Worthington Regional Hospital Home Care was selected to participate in the Minnesota Home Health Collaborative Pilot Mentoring Program to promote home health quality improvement. Stratis Health, Minnesota's Medicare Quality Improvement Organization, implemented the program under a contract with the Centers for Medicare & Medicaid Services.

The mentoring program offered agency staff the opportunity to network with other professionals across agencies and to work collaboratively to learn new ways to solve problems and improve patient outcome rates for avoidable acute care hospitalizations and/or oral medication management.

Stratis Health matched agencies that could benefit from additional assistance with agencies that had already achieved success in both or either area.

"Other home care agencies understand the challenges unique to providing services in the home care setting," said WRH Home Care Manager Jane Skaar. "Both agencies were open to discussion as to how specific interventions could be applied in each agency. Talking to another agency helped to put our struggles in perspective and we were able to share ideas and resources on how to improve our patient outcomes."
